Score: 88, Grade: B
Violations: Observed fume hood with grease build up. PIC will inform owner. New Violation. Observed baffle inside ice machine needs cleaning; PIC will have baffle cleaned. New Violation. Observed prepared ready to eat food not marked with date of preparation or date for disposal. Discussed proper date marking procedures with PIC. Repeat Violation. Observed build up of dust on ceiling tile and vents over cook area; PIC will inform owner. Repeat Violation. Observed floor tile in areas needs replacing; PIC will have tiles replaced New Violation. Observed vent on wall near backdoor and in customer restroom needs cleaning; PIC will have vents cleaned. New Violation.

Violations: Observed the fume hood needs to be cleaned. Discussed cleaning frequency of the fume hood with PIC. Repeat Violation. Correct by Nov. 9. Observed ants in the main lobby. PIC will contact the exterminator to spray the facility. New Violation. Correct by Nov. 9.

Violation: Observed the outside waste receptacle lid was not closed during a period of inactivity. Discussed with CFSM. Corrected on site by having his employee close the lid. Corrected On-Site. Repeat Violation.
Remarks: Discussed routine maintenance and sanitizing of ice machine. Discussed covering foods i.e. steam well items when not in use. Discussed cooler thermometers.

Score: 96, Grade: A
Violations: Observed facility without a copy of the vomiting/fecal cleanup procedure. PIC was provided with a copy of the procedure. Corrected On-Site. New Violation.
Remarks: Discussed placing in use utensils in warm water at 135 degrees Fahrenheit or on a clean food preparation table or cooking utensil that is cleaned and sanitized frequently.

Violations: Observed pizza station reach in cooler with prep-top cold holding potentially hazardous food at greater than 41 degrees Fahrenheit. Discussed with Nick and Billy. Billy placed call requesting cooler to be serviced/repaired. Discussed with PIC Nick to have potentially hazardous foods discarded. New Violation. Correct by Nov. 3.
Remarks: Discussed freezer door difficult to latch. Discussed relabeling any items not in their original containers if labels become worn and/or difficult to read.

Remarks: Recommend moving single service items away from exposed plumbing under hand sink. Discussed light requirements in restrooms.
