Observed food stored on floor and shelving not allowing a 6-inch clearance from off floor (drinks, water, rice, boxes of soy sauce, duck sauce); PIC remove food items and place on adequate shelving. New Violation. Observed female employee handling on preparing food not wearing a restraint. PIC had employee put on a hair restraint. Corrected On-Site. New Violation. Observed food prep cooler cutting board not maintained in good repair; PIC will replace cutting board. New Violation. Correct by March 8. Observed bowl being used as a food scoop in food containers; PIC remove bowl. Corrected On-Site. New Violation. Hot water supply not maintained during peak periods; PIC will have someone come and check it out to have repaired. New Violation. Employee to educate staff regarding routine cleaning/sanitizing of ice machine chute. New Violation. Correct by Feb. 29. Observed QAC test strips not on hand. Discussed with CFSM who will purchase new test strips. Three-compartment sink tested with EH test strip correctly at 200 ppm. QAC sanitizer bucket also tested with EH test strip correctly at 200 ppm. She was instructed to throw out all food not marked and start hand writing time tickets until machine was fixed. Corrected On-Site. New Violation.

LEESBURG EXPRESS/KRISPY CRUNCHY, 275 U.S. Highway 19 South, Leesburg

Violations: Observed raw animal food stored over ready-to-eat food in bottom prep top cooler. PIC corrected on sight by moving ready-to-eat foods in the top storage compartment of unit. Corrected On-Site. New Violation. Cooked meats or poultry not held at 135 degrees Fahrenheit or above. Observed corn dogs and fried okra held at 105 degrees Fahrenheit under heat bulbs. PIC corrected by discarding all food under bulbs. Advised PIC to change bulbs or have unit serviced. Corrected On-Site. New Violation. Hot water not provided/shut off at employee handwash sink. Advised PIC to not use handwash sink until hot water was restored and use handwash sink in food prep area that did have hot water. New Violation. Correct by March 7.

Remarks: Discussed having prep top cooler serviced due to border-line temps in the cooler. Discussed importance of correct single-service item storage.

Remarks: Discussed with PIC about proper postings, proper ware washing procedures, and gave forms for cleanup procedure and employee reporting agreement. Discussed change of ownership with PIC.
