Violations: Observed wiping cloths not held in a chemical solution at the proper concentration of 200 ppm QAC. PIC changed solution in the sanitizer bucket. Corrected On-Site. New Violation. Observed gaskets on front line cold units in need of repair. PIC will have gaskets repaired or replaced. New Violation. Observed leak under fountain drink machine. PIC will have leak repaired. New Violation. Observed hole in the wall near three-compartment sink. PIC stated maintenance workers were repairing AC unit and they had to go through the wall to reach the unit. AC unit is still being repaired. PIC will have the hole repaired once maintenance work is done. New Violation.
Remarks: Facility has a cleanup kit.

Observed men’s restroom handwashing sink not provided with a supply of hand cleaning liquid, powder or bar soap. PIC will have handwashing sink supplied with soap. New Violation. Observed men’s restroom handwashing sink not provided with individual disposable towels. PIC will have handwashing sink supplied with paper towels. New Violation. Observed handwashing sink with utensils and equipment in basin. PIC removed utensils from handwashing sink. Corrected On-Site. New Violation. Observed utensils stored in food with their handles in contact with the food. PIC removed utensils. Corrected On-Site. New Violation. Observed boxes of plastic single-service and single-use articles not stored at least 6 inches above the floor. PIC will store articles at least 6 inches off the floor. New Violation.
Remarks: Discussed removing items not necessary to the operation of the facility.

Violations: Observed few styrofoam cups not stored covered in a manner to prevent hand to lip contact. Discussed with Mr. Magaddio and waitresses proper way to store them. New Violation.
Remarks: +/-3 degrees Fahrenheit allowed for thermometer accuracy. Work order out for Echolab dishwasher maintenance. Employee using three-compartment sink properly for wash, rinse and sanitize. Few chlorine test strips given to test three-compartment sink to be at 100 ppm by EH until Echolab arrives. New ice machine purchased.
