Notes: Observed raw fish stored on the same shelf as commercially prepared and ready to eat whipped cream. Person in charge moved the whipped cream to a higher level shelf with the other ready to eat foods. Corrected On-Site. New Violation; Observed potentially hazardous food (rice and macaroni & cheese) cold holding in small upright refrigerator above 41 degrees Fahrenheit. Person in charge discarded rice and notified to discard macaroni and cheese. New Violation; Observed raw chicken prepped in bus tubs and stored inside the cook line cabinet prep cooler cold held above 41 degrees Fahrenheit. Person in charge put ice on the chicken to cool it down and called the refrigerator service provider to repair. New Violation; Observed cooked macaroni and cheese and cooked cold held for 24 hours without a date of discard. Person in charge notified to put an expiration date on the macaroni and cheese. The rice was discarded. New Violation; Observed condensate drainage from reach in refrigerator dripping on wrapped individual cooking portions of chicken livers and gizzards stored in containers. Person in charge drained the water out of the containers, moved the portions to clean containers and covered the containers. Corrected On-Site. New Violation; Observed single service articles not properly protected while on display and in use on the serving line. Corrected On-Site. Repeat Violation; Observed small upright reach in refrigerator condensate draining inside the refrigerator. New Violation.

Notes: Observed multiple potentially hazardous food items being held above 41 degrees Fahrenheit in cold holding drawers. PIC had items removed from service and put in order to have coolers repair. Repeat Violation; Observed the vegetable sink being used to wash hands. Discussed only using vegetable sink for washing fruits and vegetables with PIC. New Violation; Observed the wrong inspection score posted and no score posted at the drive thru. Advised PIC to post most recent score and post score at drive thru. Repeat Violation; Observed the vent on the fan in the walk in cooler is missing. PIC will put in order to have vent replaced. New Violation; Observed walls in walk in cooler with accumulations of dust. Discussed cleaning with PIC. New Violation; Observed vents in ceiling and vent in walk in freezer in need of cleaning. Discussed routine cleaning with PIC. Repeat Violation.

New Violation; PIC unable to locate a written employee health policy or verbalize restrictive symptoms or illnesses. PIC was unfamiliar with illnesses that should be reported. New Violation; Observed nonfunctioning automatic paper towel dispensers in men’s and women’s restrooms. COS-PIC had employee replace battery and properly load towels into dispensers. Corrected On-Site. New Violation; Observed raw shell eggs stored on top of cooking equipment at 47 degrees F. Observed carton of egg white product stored on top of cooking equipment at 77 degrees F. COS-Employee advised shell eggs had been out of cold storage for 10-15 minutes so they were placed back into cold holding unit. The carton of egg white product had been out for an undeterminable amount of time and were discarded by PIC. Corrected On-Site. New Violation; Observed disposable drinking cup used to scoop ice in ice bin in drive thru area. New Violation; Observed clean and sanitized utensils stored in drawer with visible food debris. New Violation; Drainboard was not large enough to accommodate all dirty dishes and utensils. Observed drainboard full of dishes and a pan of dirty dishes stacked on top of the mop sink. COS-PIC rearranged items on drainboard and moved the pan of dishes from the mop sink to the drainboard. Corrected On-Site. New Violation; Observed end of hose stored below flood rim of mop sink. COS-PIC properly stored hose with end above flood rim of mop sink. Corrected On-Site. New Violation.
