Score: 96 Grade: A

Violations: Observed the facility did not have a copy of the employee health policy available. PIC was given a copy of the policy. Corrected on-site, new violation. Observed the facility did not have a copy of the vomiting/fecal cleanup procedure available. PIC was given a copy of the procedure. Corrected on-site, new violation.

Shogun Japanese Restaurant, 629 N. Westover Blvd. Suite B

Score: 95 Grade: A

Violations: Observed some areas inside ice machine not clean to sight and touch. Discussed with CFSM. CFSM to have ice machine cleaned and sanitized and ensure routine cleaning and sanitizing schedule remains in place. New violation. Observed broken tiles in dishwashing room. Discussed with CFSM. CFSM/owner to have tiles repaired/replaced by 3/5/18. New violation.

Score: 98 Grade: A

Violations: Observed dishes stacked wet. Discussed with PIC how to stack washed items to air dry. PIC to restack dishes to encourage air drying. Repeat violation.

Violations: Observed light bulbs in the food prep area that were not shielded, coated or otherwise shatter-resistant. Discussed with PIC. PIC will replace the shield on lights. Repeat violation.

Score: 92 Grade: A

Violations: Observed fume hood needs cleaning; PIC will have fume hood cleaned. Repeat violation. Observed floor tile in dish area and throughout areas in kitchen need repairing. PIC will have tile repaired. Repeat violation.

Violations: Observed no thermometer in reach-in cooler located under prep-top cooler. Discussed with PIC. PIC to replace missing thermometer. New violation.

Score: 93 Grade: A

Violations: Observed interior of microwave needs cleaning; PIC had employee wipe down interior of microwave with wiping cloth. Corrected on-site, repeat violation. Observed exteriors of drink machines in lobby and drive-thru need cleaning; employee cleaned drink machines with wiping cloth. Corrected on-site, new violation.

PIC relocated all TCS foods to properly functioning cold hold units until non-functioning units can be repaired. Corrected on-site, repeat violation. Shredded cheese, sliced cheese held longer than 24 hours with no throw-out date marked. Pork barbecue held past throw-out date marked. EHS instructed PIC to label cheese and discard barbecue. Repeat violation. Bleach, soap stored above onions, potatoes in dry storage area. PIC separated cleaning supplies from food products in storage. Corrected on-site, new violation.
