Observed cardboard on floor in kitchen; cardboard was removed. New violation. Observed ceiling tile peeling in certain areas in kitchen; PIC will have ceiling repaired. New violation.

Violations: Observed employees not washing their hands in a handwashing sink or approved automatic handwashing facility, but cleaning their hands in a food preparation and warewashing sink. Discussed with PIC about designating a sink for handwashing. New violation. Observed no soap or paper towels at the sink that should be designated as the handwashing sink. New violation. Observed no hot or cold water generated to meet the demand in the employees’ restroom or the handwashing sink in the main kitchen. A work order will be placed to have the water fixed. Repeat violation.

Discussed with PIC to check and ensure staff checks sanitizing mixture routinely with the QAC test strips observed to be on hand. Repeat violation.
