Steak & Shake, 718 N. Westover Blvd.

Notes: Observed utensils being washed in the mechanical ware washing but no chlorine sanitizer was being dispensed to sanitize the utensils. PIC contacted the repairman and the equipment will be serviced today; repeat violation. Observed TCS foods being held above 41 degrees Fahrenheit in prep top coolers. See temp log. PIC discarded items; corrected on-site, repeat violation. Observed TCS food sausage patties being held below 135 degrees Fahrenheit on the steam table. PIC discarded items; corrected on-site, new violation.

Notes: Observed cases of beverages stored on floor less than 6 inches from floor; new violation. Observed single-service articles not properly protected from potential contamination during storage while on display; new violation. Observed floor-mounted equipment not elevated on 6-inch legs, installed on castor wheels or sealed permanently to the floor; new violation. Observed utensils near the three-compartment sink that were stacked directly while wet so that air drying was not encouraged. PIC trained dish washing employee on proper stacking; repeat violation.

Observed the interior of the reach-in freezer in the back near dry storage was not clean to sight and touch. PIC will have the freezer cleaned; new violation. Observed a single-use article being reused. PIC discarded the item; corrected on-site, new violation.

Notes: Observed no hand-cleansing agent provided at the hand sink in the kitchen (due to dispenser not working). PIC will have hand soap dispenser repaired; new violation. Observed TCS food items in the walk-in cooler that had been held for more than 24 hours that were not dated with the date the item should be discarded. PIC removed items; corrected on-site, new violation.

Notes: Observed PIC was not able to verify the diseases and symptoms associated with foodborne illness with a written copy of the employee health policy. Gave PIC another copy of the employee health policy; corrected on-site, new violation. Observed wiping cloths that were not stored in chemical sanitizer solution at the proper strength during periods of non-use. PIC remade sanitizer at 200 ppm; corrected on-site, new violation.

PIC re-stacked items; corrected on-site, new violation.

Observed wiping cloths stored in water that contained no chlorine sanitizer. Employee made fresh sanitizer at 100 ppm; corrected on-site, repeat violation. Observed wiping cloths that were not stored in chlorine sanitizer solution during periods of non use. PIC placed cloths in sanitizer solution; corrected on-site, new violation.
