Chick-Fil-A, 2703 Dawson Road

Notes: Observed utensils directly stacked while wet so air drying was not encouraged. PIC restacked items. Corrected on-site, new violation; Observed drain plug on Dumpster is broken so the container is not leak-proof. PIC will have drain plug replaced. New violation.

Dairy Queen, 1804 E. Ogelthorpe Blvd.

Notes: Observed fume hood needs to be cleaned. PIC will have fume hood cleaned. New violation.

Starbucks Coffee, 2709 Meredyth Drive, Site 120

Notes: Observed the mechanical dish washing machine not reaching the minimum hot water sanitization temperatures per the data plate. PIC will have dish washing machine serviced. New violation.

T-Bones Wings & Things, 1905 S. Madison St.

Notes: Observed QAC sanitizer concentration at 50 ppm in the three-compartment sink. QAC chemical sanitizer concentration is required to be 200 ppm. PIC remade chemical sanitizer solution. Corrected on-site, new violation.

Notes: Observed hand-washing sink in the kitchen was not supplied with hand drying provision. PIC placed paper towels at the hand sink. Corrected on-site, new violation; Observed bulk containers of corn starch, salt, flour and rice not correctly labeled with the common name of the food. PIC will have containers labeled today. New violation; Observed containers of soy sauce that were not stored 6 inches off the floor. Discussed purchasing adequate shelving to store all items off the floor with PIC. New violation; Observed employee in the kitchen cooking without a hair restraint. PIC had employee put on a hair restraint. Corrected on-site, new violation; Observed wiping cloths stored in water that contained no sanitizer solution during periods of non use. PIC made fresh sanitizer. Corrected on-site, new violation; Observed raw meat being washed in the sink designated for washing fruits and vegetables only. Discussed using vegetable sink for only washing vegetables fruits and vegetables. New violation.

Notes: Observed no paper towels at the employee hand-washing sink. PIC will place paper towels at the sink. New violation.

Papa John’s Pizza, 711 N. Westover Blvd., Site G

Notes: Observed no evidence posted for a Certified Food Safety Manager in the establishment. PIC stated he will be taking the class this month. New violation; Observed wiping cloths stored in solution that was not at the required sanitizer concentration during periods of non use. PIC made fresh sanitizer. Corrected on-site, new violation.

Notes: Observed TCS food not cooled to 41 degrees Fahrenheit or less within 4 hours. PIC removed tomatoes from service. Corrected on-site, new violation.

Notes: Observed fume hood needs to be cleaned. PIC will have fume hood cleaned. New violation; Observed the interior of the microwave needs to be cleaned. PIC will have the microwave cleaned. New violation; Observed inspection report not posted at the drive-through window. PIC will post inspection at window. New violation; Observed no hot water at the vegetable sink. PIC called maintenance to fix the hot water at the vegetable sink. New violation.

Notes: Observed that the fume hood needs cleaning. PIC will have fume hood cleaned. New violation.

Big Rax Saloon, 717 N. Westover Blvd., Site E, F, G

Score: 86

Notes: Observed no proof of a CFSM in the establishment. The facility will have six weeks to obtain a CFSM by 2-19-16. Repeat violation; Observed that hand sink was left inaccessible with paper plates and other material being stored in the sink. PIC removed items from hand sink. Corrected on-site, new violation; Observed food items — vegetable balls, rice and curry vegetables stored in the walk-in cooler that was not in a covered container. Discussed covering food with the PIC. New violation.

Casa Tapatia Mexican Resturan, 108 N. Slappey Blvd.

Notes: Observed ice stored uncovered in ice bins at the bar and in the main kitchen. PIC closed ice bin covers. Corrected on-site, new violation; Observed chlorine chemical sanitizer concentration too high in the mechanical warewashing machine. Chlorine concentration was over 200 ppm. Proper concentration is 50-100 ppm. PIC will have sanitizer system recalibrated to dispense the proper concentration of sanitizer. New violation; Observed TCS foods (beans and ground beef) held above 41 degrees Fahrenheit in the walk-in cooler. PIC will have items discarded. Corrected on-site, new violation.

Shiawase Hibachi & Sushi, 1210 A U.S. Highway 19 S.

Score: 88

Person in charge could not demonstrate knowledge of employee health symptoms as it relates to diseases that can be transmitted through food. New violation; Observed uncovered/open bags of rice stored in dry storage area when not in use. Manager corrected violation by covering/closing bags around rice. Corrected on-site, new violation; No consumer advisory on consuming raw fish posted on sushi menu. New violation.
