Mellow Mushroom Pizza Bakers, 2825 Nottingham Way

Notes: Observed TCS foods being held in the reach in coolers in pizza and salad prep areas that were not being maintained at 41 degrees Fahrenheit. See temp log. PIC removed the items. Corrected On-Site. New Violation; Observed ice used as an ingredient in drinks that were also being used to as an exterior coolant for drinks. Discussed with PIC that ice used as a coolant is prohibited as an ingredient. New Violation; Observed an employees personal food items stored directly on top of food items for the facility. PIC removed the personal items. Corrected On-Site. New Violation; Observed utensils in buffalo chicken dip and blue cheese dressing that were not stored with the handle above the food. PIC removed the utensils. Corrected On-Site. New Violation; Observed utensils on a rack near the dishwashing area that were directly stacked while wet so that air drying was not encouraged. PIC had employee re-stack items. Corrected On-Site. New Violation.

Notes: Potentially hazardous food (BBQ chicken and smoked turkey) not held at 135 degrees Fahrenheit or above. PIC reheated BBQ chicken to proper temperature and discarded smoked turkey. Corrected On-Site. New Violation; Food found stored past disposal date marked. Observed tuna salad and crab salad with a discard date of 10/3. These items were discarded by PIC. Corrected On-Site. New Violation; Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. PIC discarded and made sanitizing solution at 200 ppm. Corrected On-Site. New Violation; Observed faucets in dish wash area and women’s bathroom is disrepair (running, unable to shut off). Must correct by 11/18/16. New Violation; Observed damaged floor tile in dish wash area. Discussed repairing by 11/18/16. New Violation; Observed floors with built up debris and trash below equipment in grill area. Notes: Observed the hand sink contained ice from a drinks that had been poured in the sink. Discussed that hand washing facilities may not be used for purposes other than hand washing with the PIC. New Violation; Observed the hand washing sink at the satellite concession stand was not supplied with hand drying provision. Discussed keeping all hand sinks fully supplied with PIC. New Violation; Observed the hand sink in the satellite concession on the right side of the building was not supplied with hot water under pressure. Discussed having sink repaired with PIC. Repeat Violation; Observed light bulbs in the food prep areas that were not shielded, coated, or otherwise shatter resistant. PIC will have shields placed over the lights that were missing them. New Violation.
