The Mayberry Diner, 2349 Lake Park Drive

Notes: Observed items in the walk-in cooler that were not being maintained at 41 degrees Fahrenheit for cold holding. Employee discarded items; corrected on-site, new violation. Observed a utensil used for scooping in a bulk bin of sugar that was not stored with the handle above the food. PIC will replace the utensil with one that has a proper handle; new violation. Observed an accumulation of dust on air conditioning vent in the kitchen. Employee removed the vent and cleaned it; corrected on-site, new violation. Observed that the light bulb in the walk-in cooler was not shielded, coated or otherwise shatter resistant. PIC will purchase a shield for the light bulb, new violation.

Notes: Observed ants in the storage room. Discussed pest control frequency with the PIC; new violation.

Notes: Observed no written policy in the establishment to advise employees how to respond to vomit/fecal incidents. Gave PIC another copy of the vomit/fecal incident clean-up procedure; corrected on-site, new violation. Observed raw fish that was not stored at least 6 inches above the floor in the walk-in cooler. PIC will rearrange items in freezer so that all items are stored 6 inches above the floor; new violation. Observed the receptacle in the unisex restroom was not provided with a cover. PIC will have a cover placed on the receptacle; new violation. Observed lights in food preparation areas that were not shielded, coated or otherwise shatter resistant. PIC will have lights shielded; new violation.

Notes: Observed that raw and cooked ready-to-eat foods were not separated from raw animal foods so that cross contamination of one type with another was prevented. PIC rearranged items; corrected on-site, repeat violation. Observed food items stored in the reach-in cooler where they could be exposed to contamination by employees’ personal food items. PIC moved the employees’ personal items; new violation. Observed employees preparing egg rolls that were not using effective clean, disposable or easily cleanable hair nets or other hair restraints to properly retrain loose hair. PIC will ensure that all food prep employees wear hair restraints; new violation. Observed scooping utensils in bulk food bins that were not stored with the handle above the top of the food. PIC stored utensils with the handle above the top of the food; corrected on-site, new violation. Observed utensils near the three-compartment sink that were stacked directly while wet so that air drying was not encouraged. PIC re-stacked items; corrected on-site, new violation. Observed hot water under pressure was not being supplied to the sink in the employee restroom, and cold water under pressure was not being applied to the three-compartment sink. PIC will have a plumber check both sinks, new violation.

Notes: Observed containers of milk in the walk-in cooler that were not stored at least 6 inches above the floor. PIC stored items 6 inches above the floor; corrected on-site, new violation. Observed cloths for wiping counters and other equipment surfaces that were not held between uses in chemical sanitizer solution. PIC placed the cloths in chemical sanitizer solution; corrected on-site, new violation. Observed slicing utensils being soaked in the sink that is designated for washing fruits and vegetables only. PIC removed the utensils; corrected on-site, new violation. Observed dishes directly stacked while wet near the three-compartment sink so that proper air drying was not encouraged. PIC informed workers to stack dishes indirectly in the future; repeat violation.

Notes: Observed that the facility did not have in its employ a certified food safety manager to ensure food safety is being managed within the food service establishment due to the previous CFSM no longer being employed. The establishment will have 60 days to obtain a new CFSM; new violation. Observe TCS food items in the refrigerator that were not being maintained at 41 degrees Fahrenheit or below. PIC removed items; corrected on-site, new violation. Observed light bulbs in the food prep area that were not shielded, coated or otherwise shatter resistant. PIC will have shields placed over bulbs; new violation. Observed the presence of insects was not controlled to minimize their presence on the premises. Observed live roaches in the kitchen near the hand-washing sink. PIC will have pest control treat the facility; new violation.

Dunkin Donuts, 2409 Dawson Road

Notes: Observed pans and plastic containers stacked wet. Discussed proper stacking with PIC. PIC staked dishes to allow them to dry; corrected on-site, new violation.

Locos Grill & Pub, 547 N. Westover Blvd.

Score: 91

Notes: Observed an employee in the kitchen drinking from a single-service cup that was not secured with a tightly fitting lid and straw. Employee discarded drink; corrected on-site, new violation. Observed utensils being washed in the ware washing machine, but no chlorine sanitizer was being dispensed to sanitize the utensils. PIC repaired dish machine and it began to dispense chlorine sanitizer at 100 PPM; corrected on-site, new violation. Observed a utensil used for scooping in a bulk sugar bin that did not have a handle to be stored above the top of the food. PIC removed the utensil from the bin; corrected on-site, new violation.
