Violations: Observed no written policy to instruct employees how to respond to vomit/fecal incidents involving surfaces in the facility. Will give PIC another copy of vomit/fecal incident clean up form. Repeat Violation. Observed the fans on the interior of the walk in cooler were not free from accumulations of dust. PIC will have walk in cooler fans cleaned. New Violation. Observed the light bulbs exposed to food in the walk in freezer and food prep areas were not properly shielded, coated, or otherwise shatter resistant. Light must be properly shielded by May 17. Repeat Violation. Correct by May 17.

Violations: Observed no soap at the employee’s hand washing sinks. PIC replace the containers at the sinks. Corrected On-Site. New Violation. Discussed the frequency of cleaning with the PIC. New Violation.

Violations: Observed a bin of flour stored under the three compartment sink near cleaning chemicals where it was exposed to contamination. PIC had the flour discarded. Corrected On-Site. Observed the fan and rack in the reach in cooler that were not free from accumulations of dust. PIC will have fan and racks cleaned. New Violation. Observed an accumulation of grease in the area beside and behind the fryer. Discussed that physical facilities must be cleaned as often as necessary to be kept clean. PIC will have floor cleaned. Violations: Observed utensils being washed but not sanitized in the lounge area dishwasher due to sanitizer bottle being empty. PIC had sanitizer replaced. Corrected On-Site. New Violation.
