Score: 97, Grade: A
Violation: Observed meat thawing in the prep sink under an unapproved method. PIC placed the meat under running water. Corrected On-Site. New Violation.
Remarks: Discussed ice machine, food labels, dumpster lids.

Violation: Observed RIC for storing beverages not maintaining ambient temperature of 41 degrees Fahrenheit. PIC will have the cooler serviced. New Violation. Observed women’s restroom is not equipped with a self closing door. PIC will fix the restroom door. New Violation.
Remarks: No PHF prepared at the facility. PIC will have 10 days to get the RIC serviced (July 21,2017).

Violations: Observed nozzles on the drink machine were not clean to sight and touch. PIC removed nozzles and cleaned them. Corrected On-Site. Repeat Violation. Observed a bottle containing toxic chemicals that was not in an area that was not above food, equipment, and single use articles. PIC moved the bottle of chemicals to the proper storage area. Corrected On-Site. New Violation.
Remarks: Discussed that PIC will purchase a brush for scrubbing drink nozzles. Discussed food and chemical storage.
Coach’s Sports Bar LLC, 2610 Dawson Road, Suites 10-11
Score: 92, Grade: A
Violations: Observed no CSFM certification displayed and no documentation found of certification on hand. Discussed with Manager. New Violation. Observed no paper towels at hand washing sink at bar and in main kitchen. Discussed with Manager and corrected on site. Corrected On-Site. New Violation.
Remarks: Given Employee Illness Quick Decision Guide and Manager posted it on bulletin board with Employee Health Policy and Clean Up Procedure Vomit/Fecal Events.
Hello Smoothie, 1019 N. Westover Blvd.
Score: 97, Grade: A
Violation: Observed cloths for wiping counters and other surfaces that were not stored in chemical sanitizing solution. Discussed with CSFM. CSM placed clothes in QAC chemical sanitizing solution 200 ppm. New Violation.
Remarks: Discussed with CSFM proper stacking of just washed, rinsed and sanitized containers to allow proper air drying. CSFM mentioned that they may start serving sandwiches in about 2 weeks. Discussed with him that EH will need to see a menu of what new items they plan to serve so their kitchen may be evaluated to ensure proper equipment is present for the menu additions.

Violations: Observed the food service establishment without a valid food service permit from the Health Authority. PIC will contact the home office to obtain the food service permit. New Violation. Correct by July 14. Observed single service articles not properly protected from contamination when on display and in use. New Violation. Observed the area over the fryer not free of accumulation of soil residue. PIC will have the area over the fryer cleaned. New Violation. Correct by July 14. Observed floor tile damaged or missing. PIC will have the tiles replaced. New Violation. Observed light bulbs without shield or coating in the food preparation area. PIC will have the bulbs covered. New Violation. Correct by July 14.
Remarks: Manager prepared corn 1 hour before the inspection began. The temperature of the corn was at 55 degrees at 1:45pm and had cooled 8 degrees in 30 minutes therefore, it would meet the cooling requirement of 41 degrees or below within 6 hours. The PIC will have until July 14 to correct the permit, single service use, and non contact food service violation. Discussed the Key Drop Delivery procedure with the PIC.
Mikata Japanese Steak House, 2610 Dawson Road
Score: 96, Grade: A
Violation: Observed cloths for wiping counters and other surfaces in the facility in chemical sanitizer of Clorox that was not within range (10 ppm). CSFM notified and corrected on site. Clorox sanitizing level raised to 100 ppm. CSFM to check and ensure proper sanitizing levels routinely. Corrected On-Site. Repeat Violation.

Violations: Observed personal medicine stored in refrigerator and not properly separated; medicine was removed from refrigerator. Corrected On-Site. New Violation. Observed personal drinks not properly stored in reach in freezer; Drinks were removed from reach in freezer. Corrected On-Site. New Violation.

Score: 97, Grade: A
Violations: Observed no hot water at the 3 compartment sink behind the bar. PIC will have the water fixed. New Violation. Observed no self closing device on the women and men restroom doors. PIC will replace the closers. New Violation. Observed no trashcans available in the women and men restrooms. PIC will be placing trashcans in the restrooms. New Violation. Observed no drain plug at the dumpster. PIC will have the plug replaced. New Violation.
Remarks: No PHF being prepared at this facility. Discussed sanitizer strips to test sanitizer at the 3 compartment sink. PIC will submit specifications for a small cooler to Environmental Health for review before placing it in the facility. PIC was given a copy of the employee health policy and vomiting/fecal cleanup procedure.

Downtown Southern Kitchen/Vintage Haven, 230 W. Broad Ave.
Score: 81, Grade: B
Violations: Observed no paper towels at the employee hand sink in the kitchen. PIC placed paper towels at the sink. Corrected On-Site. New Violation. Observed no soap at the employee hand washing sink in the kitchen. PIC placed soap at hand sink. Corrected On-Site. New Violation. Observed accumulation of grease on the fume hood. Discussed frequency of cleaning with PIC. Repeat Violation. Observed TCS food items in the Walk In Cooler being held above 41 degrees Fahrenheit. PIC discarded the food items. New Violation.
Hibachi Express, 2901 Meredyth Drive
Score: 94, Grade: A
Violation: Observed no detectable chlorine sanitizer at 3 compartment sink. Noted sink plugs in 3 compartment sink missing and drains plugged with paper towels. PIC notified and agreed to call owner and have plugs replaced then bring sanitizing solution up to minimum of 100 ppm. PIC to test sanitizing solution at 3 compartment sink routinely to ensure that it stays within proper range parameters. Repeat Violation. Correct by July 23.
Remarks Addendum: July 13 3:20 pm Call placed to owner Terry Ho and discussed the 3 compartment sink and issue of no detectable sanitizing solution as well as missing plugs. He stated that PIC Ms. Taylor had already called and explained the situation and that he has already ordered the new sink plugs that should arrive in approximately 2-3 days. We discussed this needs to be remedied within 10 days. In the mean time, he plans to set up sanitizing solution in a container within correct level range to ensure. Proper sanitizing.
