These establishments were inspected June 10-16 by the Dougherty, Lee, Worth, Baker, Terrell, Mitchell and Calhoun County Environmental Health offices. To contact the health office, call (229) 438-3943.

DOUGHERTY

June 12

Bruster’s, 2822 Old Dawson Road

Score: 94, Grade: A

Violations: Observed no written policy for instructing employees on foodborne illness reporting responsibilities and proper restriction and exclusion. Gave PIC another copy of the employee health policy. Corrected On-Site. New Violation. Observed no hand drying provision provided at the employee hand washing sink. PIC placed paper towels at the sink. Corrected On-Site. New Violation. Observed spoons that were not commercially pre-wrapped that were not presented so that only the handles are touched by employees. PIC rearranged the spoons to protect food- and lip- contact surfaces. Corrected On-Site. New Violation. Observed missing and broken tiles in the back prep area. PIC will have tiles repaired. New Violation.

Remarks: Facility was undergoing a thorough cleaning during inspection. Gave PIC a copy of the employee health policy and vomit/fecal incident clean up form. Advised PIC that annual fee will be due immediately.

Score: 93, Grade: A

Violations: Observed interior of microwave needs cleaning; PIC had employee wipe down interior of microwave with wiping cloth. Corrected On-Site. Repeat Violation. Observed fume hood above grill needs cleaning; PIC will have fume hood cleaned. New Violation. Observed wet stacked food containers; Discussed air drying with PIC New Violation.

House of China II, 2526 Dawson Road, Suite A

Score: 80, Grade: B

Violations: Observed employee drinks that were not in single service cups that were secured with a tightly fitting lid and straw. PIC had lids placed on the drinks. Corrected On-Site. New Violation. Observed TCS items in the walk in cooler that were not being maintained at or below 41 degree Fahrenheit for cold holding. See temp log. PIC removed items from the walk in. Corrected On-Site. New Violation. Observed TCS food ground beef in the walk in cooler that had been cooked and held in the walk in cooler for over 24 hours that were not labeled with the date the item should be discarded. PIC removed the items. Corrected On-Site. New Violation. Observed cloths for wiping counters and other surfaces in the facility that were not stored in chemical sanitizer solution between uses. PIC had cloths placed in sanitizer. Corrected On-Site. New Violation.

DJ’s Lounge & Grill, 1001 Radium Springs Road

Score: 98, Grade: A

Violations: Observed no drain plug on the dumpster. PIC will contact Waste Industries to get a drain plug. Repeat Violation. Observed inadequate lighting in the food prep area. PIC will replace the light bulbs in the kitchen. New Violation.

Fairfield Inn/Food Service, 3011 Kensington Court

Score: 91, Grade: A

Violations: Observed foods boiled eggs, shredded cheese and butter in ice baths being held above 41 degrees Fahrenheit. PIC discarded the items. New Violation.

Remarks: Plus/minus 3 degrees Fahrenheit for temperature accuracy. Discussed with CFSM to make frequent temperature checks of ice bath items to ensure potentially hazzardous foods are held at 41 degrees fahrenheit or below. Discussed with her ServSafe expiration date of Sept. 30.

Taco Bell, 2414 Dawson Road

Score: 100, Grade: A

June 16

Shogun Japanese Restaurant, 629 N. Westover Road, Suite B

Score: 87, Grade: B

Violations: Observed TCS foods in the reach in coolers in the front food service area that were not being maintained at 41 degrees Fahrenheit. PIC removed the items. Corrected On-Site. New Violation. Observed scoops in bulk containers with handles that did not have a handle to be stored above the top of the food. PIC removed the scoops. Corrected On-Site. Repeat Violation. Observed single use articles being reused for food storage. PIC transferred the food to reusable food grade containers. Corrected On-Site. New Violation. Observed the service was sink was not being used for the cleaning of mops and disposal of mop water or similar liquid waste. PIC made the mop sink accessible again. Corrected On-Site. New Violation.

Remarks: Plus/minus 3 degrees Fahrenheit allowed for thermometer accuracy. Discussed keeping a temperature log to verify that temperature requirements are met daily. Discussed that mops should not be cleaned outside with the water hose. Mops are to be cleaned at the service sink and waste water should be disposed of at the sink.

Pizza Hut, 1206 U.S. Highway 19 South, Unit A, Leesburg

Score: 99, Grade: A

Violation: No test strips provided for testing quat. ammonia sanitizer concentration in wipe cloth storage buckets. Manager corrected violation by refilling buckets with chlorine sanitizer and testing concentration with chlorine sanitizer test strips to ensure proper concentration. Corrected On-Site. Shiawase Hibachi & Sushi, 1210-A Highway 19 South, Leesburg

Score: 98, Grade: A

Violation: Latest inspection report not displayed for public view. New Violation. Correct by June 20. Observed in-use utensils for grill stored in bucket of water on floor. Water not maintained at 135 degrees F or above. Manager corrected violation by removing utensils and getting clean utensils for grill. Corrected On-Site. New Violation.

Remarks: Discussed proper storage of utensils between uses.

The Kitchen Comfort Food, 1346 U.S. Highway 19 South, Leesburg

Score: 95, Grade: A

Violations: Observed employee drink beverages improperly stored on prep table near food products. Person in charge corrected violation by moving beverages to another storage shelf away from food, utensils and single service items. Corrected On-Site. New Violation. Observed in-use utensils stored in bucket of water not kept above 135 degrees F. Person in charge corrected violation by washing utensils and storing in metal pan on stove to keep temperature of water above 135 degrees F. Corrected On-Site. New Violation.

Score: 94, Grade: A

Violations: Manager could not demonstrate knowledge of employee illness reporting requirements or exclusion/restriction policy for ill employees. Repeat Violation. Correct by June 20.

Waffle House, 1608 U.S. Highway 19 South, Leesburg

Score: 91, Grade: A

Violations: Observed raw eggs stored over an uncovered bowl of cut tomatoes in reach-in cooler near grill. Manager corrected violation by moving tomatoes to another cooler and rearranging eggs on bottom shelf of cooler. Corrected On-Site. New Violation.

Remarks: Discussed employee health policy and reporting requirements for ill employees. Discussed proper storage of raw and ready-to-eat foods in coolers.

Express Mart (Subway), 1505 Philema Road, Leesburg

Score: 81, Grade: B

Violations: Employees could not demonstrate knowledge of illnesses and symptoms to report management or an exclusion/restriction policy for ill employees Repeat Violation. Correct by June 20. Observed concentration of quat. ammonia sanitizer in 3-comp sink less than 200 ppm. Manager corrected the violation by mixing another sanitizer solution with the proper strength of 200-400 ppm. Corrected On-Site. New Violation Observed deli meat cold held above 41 degrees F (refer to temp log) in small reachin cooler behind front counter. Manager corrected violation by discarding meat. Corrected On-Site. New Violation.

Remarks: Manufactures of quat ammonia recommend a concentration of 200-400 ppm for quat to be effective at sanitizing utensils. Discussed employee health reporting requirements. Discussed repair/maintenance of reach in cooler.

WORTH

June 12

Three Squares Diner, 202 W. Franklin St., Sylvester

Score: 98, Grade: A

Violation: Observed handle of scoop stored in product in sugar container. Observed handle of scoop in product in diced ham on prep cooler. Observed handle of scoop in product in omelet mix on prep cooler. Manager moved all scoops so that handles were stored out of product. Corrected On-Site. New Violation. Observed non-food contact surface of ice machine with some buildup. New Violation.

June 13

Jackson’s Soul Food Kitchen, 113 W. Kelly St. Sylvester

Score: 98, Grade: A

Violations: Observed grease buildup on the vent hood. Observed dust buildup on the AC vent over the ice machine. Repeat Violation. Observed shield missing from the light above the coolers in the kitchen. New Violation.

Remarks: Vent hood should be serviced by professional company twice a year and the suppression system should be serviced twice a year.

Bass Markets, 103 Highway 300, Warwick

Score: 91, Grade: A

Violations: Observed hamburger steak, peas, chicken cooling in WIC covered. COS-PIC uncovered items. Corrected On-Site. New Violation. Observed frozen beef roast in pan on counter in kitchen to thaw. COS-PIC placed in RIC to thaw. Corrected On-Site. New Violation. Observed no handwashing sign posted in employee bathroom. New Violation. Observed ice scoop not properly stored. COS-PIC placed clean scoop in clean bucket. Corrected On-Site. New Violation. Observed raw wood walls/shelves in dry storage area. New Violation. Observed multiple flies in kitchen. COS-PIC closed service windows and correctly placed fly fans at service windows. Corrected On-Site. New Violation.

Remarks: Discussed hierarchy of storage, thawing procedures, cooling procedures, storage area, meat/veggie sink, wash, rinse, sanitize. NOTE: Items cooling in WIC were placed to cool at 2:50 p.m. Based on observed decrease in temp, items should reach 70 degrees F within 2 hours or less.

Violations: Food employee drinking from open cup in food preparation area. EHS instructed food employee to drink from a disposable cup with a lid and straw. New Violation. Correct by June 15. Raw shell eggs stored above, next to ready to eat foods in reach in cooler. EHS instructed PIC to separate raw animal foods from ready to eat foods in storage. New Violation. Correct by June 15.

Remarks: Items to address before next inspection: Provide CFSM by Aug. 1, hot holding temps, cold holding temps, dish machine final rinse temp, hair restraints, residential equipment.
