Violations: Observed no hand washing cleanser available at hand washing sink in upstairs and downstairs kitchen. PIC will have hand washing cleanser added to sink today. New Violation. Correct by June 1. Observed TCS Foods that where not maintained at 41 degrees for cold holding. See temp log. PIC discarded items. Corrected On-Site. New Violation.
Remarks: Discussed food storage. Discussed utensil cleaning. Discussed floor cleaning schedule. Repair man came to service cold holding equipment during inspection.
ZayBo’s Sub Shop, 2204 Palmyra Road
Score: 95, Grade: A
Violations: Observed no paper towels available at hand sink; PIC place some paper towels at hand sink. Corrected On-Site. New Violation. Observed no self closures on women and men restroom; PIC will have self closure New Violation.
Remarks: Convection oven is used in facility for baking and cooking purposes.
Zaybo’s/Mobile Unit, 2204 Palmyra Road
Score: 88, Grade: B
Violations: No Certified Food Manager for establishment. PIC was informed establishment must have a CFSM within 60 days. New Violation. Observe no paper towels at hand sink in mobile; PIC place some paper towels at hand sink Corrected On-Site. New Violation. Observed no water supply for mobile unit due to pipe not being repaired; PIC stated pipe is getting repaired. New Violation. Observed no self closures on men and women restroom; PIC will have self closures placed on restroom doors. New Violation. Observed floors need a general cleaning. PIC will have floors clean Repeat Violation.
Remarks: Mobile unit is able to be used unless water can be provided. Informed owner to change name on mobile unit to reflect new name.
June 1
Cracker Barrel, 1114 N. Westover Blvd.
Score: 90, Grade: A
Violations: Observed no written policy to demonstrate how employees comply with reporting responsibilities for food borne illness. Gave PIC a new copy of Employee Health Policy. New Violation.Observed ice machine not covered with tightly fitting lid. Discussed with PIC and covered. Corrected On-Site. New Violation. Observed dishes directly stacked while wet near the 3 compartment sink so that proper air drying was not encouraged. PIC informed workers to stack dishes indirectly in the future. Repeat Violation.
Remarks: Discussed monitoring to ensure equipment is working properly. Discussed storage of butter to ensure all products are maintained at proper temperature.

Score: 89, Grade: B
Violations: Observed fume hood needs cleaning; PIC will inform fume hood cleaned New Violation. Observed meat slicer needs cleaning to remove food debris; PIC broke meat slicer down to be clean. Corrected On-Site. New Violation. Observe chemical spray bottles not labeled. PIC labeled spray bottles. Repeat Violation. Observed floor behind and under cooking equipment needs cleaning; PIC will have floor cleaned. New Violation. Observed dust build up around ceiling vents in kitchen; PIC will ceiling vents cleaned. New Violation.
Doo-Dah’s Bar & Grill, 411 Philema Road
Score: 85, Grade: B
Violations: Observed the fume hood needs to be cleaned. Discussed frequency of cleaning with the PIC. New Violation. Observed TCS foods (slice tomatoes and slice cheese being held above 41 degrees Fahrenheit. PIC discarded the slice tomatoes and slice cheese. New Violation. Observed no hot or cold water at the 3 compartment sink behind the bar. PIC will have the water fixed at the sink. New Violation.
Phoebe Putney Healthworks/Juice Bar, 311 Third Ave.
Score: 100, Grade: A
Wing Dixie & Deli, 2601 Dawson Road, Suite B4
Score: 84, Grade: B
Violations: Observed TCS food items cold holding in prep cooler at the front of the food service that were not being maintained at 41 degrees Fahrenheit or below. See temp log. PIC removed the items. No items will be held at cold unit until it is repaired. Corrected On-Site. Repeat Violation. Observed boxes of French fries on that was not stored at least 6 inches above the floor. PIC will have employee store French fries 6 inches above the floor. Repeat Violation. Observed utensils directly stacked while wet so that air drying was not encouraged. Discussed proper stacking methods with PIC. New Violation.
Remarks: Discussed paper towels at hand sink. Discussed keeping a temperature log for prep top cooler. Discussed time as a public health control. Discussed proper sanitizing of utensils.
June 2
Ruby Tuesday, 2626 Dawson Road
Score: 87, Grade: B
Violations: Observed dishwasher was not meeting the minimum manufacturer’s requirements for hot water sanitization. PIC will have machine serviced today. New Violation. Observed TCS food items cold holding at salad bar and one reach in cooler in the main kitchen were not being maintained at 41 degrees Fahrenheit or below. See temp log. PIC removed the items. New Violation.
Remarks: Discussed dish washing machine be serviced today. Discussed wiping clothes. Discussed towels at hand sink. Discussed kitchen tiles will be replaced next week. Discussed proper handling of foods before being placed on the salad bar.
