Brother’s Pizza, 207 E. Oglethorpe Blvd.
Score: 96, Grade: A
Violation: Observed fume hood needed to be cleaned. Discussed frequency of cleaning with the PIC. New Violation.
China Garden, 2228 N. Slappey Blvd.
Score: 91, Grade: A
Violations: Observed ready-to-eat, potentially hazardous food prepared on-site and frozen not properly date marked; Discussed date marking and PIC date marked items New Violation. Observed food stored on floor and shelving not allowing a 6” clearance from off of floor; PIC remove food items and place on adequate shelving. New Violation. Observed food prep cooler cutting board not maintained in good repair; PIC will replace cutting board. New Violation. General cleaning of floors underneath fryers and wok cooking station; PIC will have floors cleaned. New Violation.

Violations: Observed employee drink uncovered in food preparation area. Discussed with PIC. PIC to have employee cover drink and ensure that other employees are made aware all of their personal beverages must be covered. New Violation. Observed one utensil near the ware washing area that was directly stacked while wet so air drying was not encouraged. Discussed proper stacking methods with PIC. PIC to have utensil restacked to encourage proper air drying. Repeat Violation. Correct by Nov. 14.

Score: 91, Grade: A
Violations: Observed Roast Beef, Mozzarella Cheese, Chicken Breast, and Steak being held above 41 degrees Fahrenheit. PIC discarded the items. Corrected On-Site. New Violation.

Violations: Observed wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength; Wiping cloth solution was refilled to proper minimum strength Corrected On-Site. New Violation.

Violation: Observed ceiling tile where vents and fan is located in kitchen and also in the men restroom needs cleaning to remove dust buildup; PIC will inform owner Repeat Violation.
Miller Brewing Cafe’, 405 Cordele Road
Score: 99, Grade: A
Violations: Observed no tight fitting lids on the dumpster. PIC will have the dumpster replaced. New Violation.
Remarks: Discussed the frequency of cleaning the grill.
Tokyo Japanese Steak House, 2724 Dawson Road
Score: 86, Grade: B
Violations: Observed the interior of the ice machine was not clean to sight and touch. Discussed with PIC. PIC to clean inside of ice machine and ensure that it is cleaned routinely as well as have routine servicing. New Violation. Correct by Nov. 17. Observed beef and scallops not held at 41 degrees Fahrenheit or below in walk in cooler. Ambient temp 48 degrees Fahrenheit as evidenced by EH thermometer reading. Discussed with PIC. PIC to have walk in cooler serviced. PIC to discard potentially hazardous foods and use reach in coolers until walk in cooler is serviced and temp of 41 degrees or below is achieved. New Violation. Correct by Nov. 17. Observed top to outside dumpster was not closed due to it was stuck on a tree branch. Discussed with PIC. PIC to close dumpster lid and ensure that dumpster is kept closed when not in use. New Violation. Correct by Nov. 17.
Remarks: Discussed maintaining thermometers in all coolers/freezers within view. Discussed keeping chlorine test strips on hand for routine testing chlorine ppm for dishwasher and sanitizing buckets. PIC to purchase chlorine test strips. Few chlorine test strips provided by EH to have on hand until purchase made. Discussed walk in cooler door handle needs repair or replacing.
Addendum: Nov. 17: After the inspection was completed, noted “Sam Service” van outside. Spoke with Sam Service employee, Scott who stated that he was in the process of servicing the walk-in cooler to bring it back to 41 degrees Fahrenheit or below and that he would put a work order in for repair/replacement of the walk in cooler door handle.

Violation: PIC could not demonstrate knowledge of illnesses and symptoms or demonstrate in a verifiable manner a method for employee reporting illnesses at time of inspection. New Violation. Correct by Nov. 20.
Remarks: Discussed importance of a written employee health policy. Manager was given copies of a example of written policy.

Little Caesar’s Pizza, 1545 Highway 19 South Suite B, Leesburg
Score: 93, Grade: A
Violations: Person in charge could not demonstrate compliance with all food employees reporting of the six illnesses and symptoms of diseases that can be transmitted through food. New Violation. Correct by Nov. 17. Observed a box of packaged cookies stored on floor behind front counter. Person in charge corrected violation by moving box to a shelf. Corrected On-Site. New Violation.
Remarks: Discussed proper storage of personal beverage cups. Discussed requirement for employee reporting agreement for illnesses. Discussed proper storage of food and single service items at least 6 inches off floor.
Sonic Drive In, 1583 U.S. Highway South, Leesburg
Score: 94, Grade: A
Violation: No disposable towels available at hand sink in food prep areas (dispenser empty). Manager corrected violation by refilling dispenser with paper towels by end of inspection. Repeat Violation.

Violations: Observed dish machine not dispensing proper concentration of chlorine sanitizer (<50 ppm) during rinse cycle. New Violation. Correct by Nov. 16. Observed box of single-service plates stored on the floor in the dry storage room. Manager corrected violation by moving the box of single-service items to a storage shelf at least 6 inches off floor. Corrected On-Site. New Violation.
Remarks: Discussed proper storage of personal beverages and belongings. Discussed proper date marking requirements and when to discard product when it is out of date.
Hot Dog King Inc., 1295 Highway 82 Unit C, Leesburg
Score: 97, Grade: A
Violation: Observed a covered plastic container of gyro meat cooling in the reach-in freezer. Manager corrected the violation by uncovering the container to allow for better heat transfer and cooling. Corrected On-Site. New Violation.
Remarks: Not enough information was gathered during the inspection to determine if cooling time and temperature was in or out of compliance. Discussed proper cooling methods to cool hot foods quickly.

Score: 92, Grade: A
Violations: No soap was provided at hand sink in food prep area at time of inspection. New Violation. Correct by Nov. 17. Observed dust and dirt on inside surfaces of vegetable prep sink (food contact surfaces). Sink not clean to sight and touch. New Violation. Correct by Nov. 17.
Remarks: Discussed cleaning and uses of vegetable prep sink. Discussed the importance of providing soap at the hand sink.
