Score: 93 Grade: A
Violations: Observed no CFSM appointed for the facility. Owner will have 60 days to obtain a CFSM. Repeat violation. Correct by Sept. 29. Observed mobile food service unit was not identified by a sign or lettering indicating the permit number. PIC will place the signage indicating the permit number within customer view. Corrected on-site, new violation. Correct by Sept. 29.

Score: 96 Grade: A
Violations: Observed no paper towels or hand cleanser at hand-washing sink. Discussed with PIC and corrected on site. Corrected on-site, new violation.
Remarks: Discussed with PIC that CSFM must obtain certification within 60 days. List of CSFM trainers and contact numbers left for Lasti Patel per her request via phone. Discussed with PIC to routinely check with test strips on hand the chlorine disinfectant used for ware washing to ensure it remains within proper range.

Violations: Observed interior of microwave needs cleaning; employee wiped down interior of microwave to remove food debris. New violation. Observed ready-to-eat, potentially hazardous food prepared on-site not properly date marked; PIC had employee properly date-mark food items. Corrected on-site, new violation.

American Legion Post 512, 815 Willie Pitts Road
Score: 94 Grade: A
Violations: Observed that the facility did not have a known certified food safety manager to ensure food safety is being managed within the food service establishment due to the previous CFSM no longer being employed. The establishment will have to obtain a CFSM within 14 days. Repeat violation. Observed at hand-washing sink that paper towels were not available. Notified PIC. Corrected on-site, new violation.
Remarks: Discussed with PIC servicing of reach-in cooler (refrigerator). Discussed with PIC importance of choosing exterminator and have routine service. PIC stated that the facility is in the process of obtaining quotes from Terminex, Adams and Astro.

Violations: Observed ranch and tarter sauces stored next to raw chicken in reach-in cooler. Items not properly separated to prevent contamination. Manager corrected violation by moving sauces to walk-in cooler. Corrected on-site, new violation. Observed cooked chicken hot held below 135 degrees F in warmer cabinet (refer to temperature log) at front counter. Manager corrected violation by discarding chicken. Corrected on-site, new violation. No test device/strips provided for testing the concentration of sanitizer in ware-wash sink. Repeat violation.
Remarks: Discussed proper storage of employee drink cups. Discussed proper display of inspection report. Discussed proper storage of cleaning chemicals between uses.

Cut melon stored next to raw beef in walk-in cooler. Cut onions stored next to raw shell eggs in walk-in cooler. Cooked chicken stored next to raw chicken in prep line cooler. Raw shell eggs stored above cooked egg rolls in prep line cooler. PIC properly separated all raw animal foods from ready-to-eat foods in storage. Corrected on-site, new violation.
Remarks: Items to address before next inspection: Proper employee drink containers, hair restraint use, walk-in cooler temperatures, residential equipment.

Violations: Observed CFSM has expired. Owner has stated that she will speak to local person about proctoring test for her so that she can meet the requirement within 60 days. Repeat violation. Correct by Oct. 1. Observed several employee drinks in the kitchen area that did not have lids and straws to prevent contamination. Owner discussed the requirement with employees. New violation.
Remarks: Discussed new rules and forms. Discussed several other items that could potentially become problems in the future.
