Notes: No written employee health policy — cardboard used as shelf/tray liner, debris on exterior of fryers, hot hold lamp & cord — leaking faucets/pipes at 3 compartment sink and bathroom handsink — bathroom not enclosed with tight fitting, self-closing door.
