DOUGHERTY

OCT. 23

Dougherty County Jail No. 120, 1302 Evelyn Ave.

Score: 91 Grade: A

Violations: Observed yellow rice, pinto beans and mashed potatoes being held at a temperature above 41 degrees. The PIC discarded the food items. Corrected on-site, new violation.

Subway, 912 S. Slappey Blvd.

Score: 94 Grade: A

Violations: Observed interior of ice machine near three-compartment sink needs cleaning; PIC will have ice machine cleaned. New violation. Observed containers stacked wet; PIC unstacked containers to allow air drying. Corrected on-site, repeat violation.

 
   

Wing Dixie & Deli, 244 Cordele Road, Suite 105

Score: 97 Grade: A

Violations: Observed previous inspection report not posted. PIC will post new inspection report. New violation. Observed no cold water at the hand-washing sink at the front counter. PIC will have the water fixed. New violation.

La Fuente Mexican Restaurant, 2000 E. Oglethorpe Blvd.

Score: 83 Grade: B

Violations: Observed no consumer advisory on the menu for rib-eye steak sold raw, undercooked or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food. PIC will inform customers that the item can only be prepared well-done. New violation. Observed refried beans and tomatoes being held above 41 degrees Fahrenheit in walk-in cooler. PIC discarded items. Corrected on-site, new violation. Observed tomatoes in the prep top cooler not cooled in 4 hours to 41 degrees Fahrenheit. PIC discarded the tomatoes. Corrected on-site, new violation. Observed taco shells and lettuce being stored on the counter near the employee handwashing sink. PIC removed the items. Corrected on-site, new violation. Observed no test kit or other device to accurately measure the sanitizer concentration in the dishwasher. PIC will purchase test strips. New violation.

OCT. 25

Arby’s No. 0686, 1105 N. Slappey Blvd.

Score: 90 Grade: A

Violations: Observed no paper towels at hand sink; PIC place hand towels at hand sink New violation. Observed containers in reach-in freezer with no lids and open bags of food exposed in walk-in freezer; PIC placed lids on containers and close bags of food. Repeat violation. Observed gaskets on drink machine at drive-thru need cleaning; PIC will have gaskets washed, rinsed, and sanitized. New violation.

BJ’s II, 2004 E. Oglethorpe Blvd.

Score: 86 Grade: B

Violations: Observed the interior of the ice machine with some mold buildup. PIC will clean the ice machine. New violation. Observed meatballs and pulled pork in the walk-in-cooler being held above 41 degrees Fahrenheit. PIC discarded the food items. Corrected on-site, new violation. Observed the exterior of the fryers not free of an accumulation of food residue and other debris. PIC will have the equipment cleaned. New violation.

Salvation Army Soup Kitchen, 304 Second Ave.

Score: 93 Grade: A

Violations: Observed expired milk in reach -in cooler. Milk was discarded. Corrected on-site, new violation. Ambient air thermometer in holding unit not accurate within +/- 3 degrees Fahrenheit. PIC will have a thermometer placed inside reach-in cooler. New violation.

Sonic Drive-In, 519 N. Westover Blvd.

Score: 94 Grade: A

Violations: Observed a cloth for wiping counters and other surfaces in the facility that was not stored in chemical sanitizer solution during periods of non-use. Employee placed the cloth in QAC sanitizer solution at 200 ppm as evidenced by test strip. Corrected on-site, repeat violation. Correct by 10/25/2017. Observed broken floor tiles in the kitchen that were collecting water and not easily cleanable. Discussed with PIC. PIC will notify supervisor/owner to have tiles repaired. Repeat violation. Correct by 11/20/2017.

Violations: Observed wiping cloth sanitizer solution not at proper strength; sanitizer solution was refilled to proper strength. Corrected on-site, repeat violation.

Score: 96 Grade: A

Violations: Observed no employee health policy for the bar. PIC was given a copy of the policy. Corrected on-site, new violation. Observed no procedure for employees to follow when responding to a vomiting or diarrheal event. PIC was given a copy of the procedure. Corrected on-site, new violation.

Hibachi Express, 1588 U.S. Highway 19 South, Leesburg

Score: 83 Grade: B

Violations: Observed employee wash and rinse utensils in three-compartment sink and not sanitize the utensils with a sanitizer solution at proper strength for the type of sanitizer to be used. Sink was not set up to sanitize utensils. Employee corrected violation by rewashing utensils using proper strength sanitizer solution. Corrected on-site, repeat violation. Observed cooked rice held in steamer at 90 degrees F. Employee corrected the violation by discarding rice. Corrected on-site, repeat violation.

Rice Bowl, 1561 U.S. Highway 19 South, Suite 5, Leesburg

Score: 91 Grade: A

Violations: Observed raw eggs stored next to and not separated from produce in walk-in cooler. Manager corrected violation by moving eggs to another shelf. Corrected on-site, new violation.

Stardust Skate Center, 2881 Ledo Road

Score: 97 Grade: A

Violations: of Code: Latest inspection report was not posted for customer view. Manager corrected the violation by posting the latest report at front counter. Corrected on-site, new violation. No hot water provided to hand sink at time of inspection. New violation. Correct by 10/30/2017.

Score: 91 Grade: A

Violations: Cooked pork links stored next to cake batter containing raw shell eggs in reach -in cooler. Raw pork loins stored next to beverages, condiments in reach-in cooler. EHS instructed PIC to separate raw animal foods from ready-to-eat foods in storage. New violation. Correct by 10/27/2017. Items to address before next inspection: Date marking for dairy products, residential equipment, repair vent hood.

New Beginnings Concessions, U.S. Highway 82 West, Dawson

Score: 82 Grade:

Violations: Raw chicken, raw beef stored above and next to ready-to-eat foods in reach-in cooler. PIC separated raw animal foods and ready-to-eat foods in storage. Corrected on-site, new violation. Beef chili (96F), cheese dip (121F) hot-held below 135 degrees Fahrenheit. EHS instructed PIC to reheat foods to 165F for 15 seconds before hot-holding. New violation. Correct by 10/27/2017. Items to address before next inspection: Illness policy, vomit policy, handwashing, wiping cloth storage. date marking, single-service and food item storage, residential equipment, employee drinks.
